Care Team Referral Form

Please complete BOTH Care Team Referral Form and Strengths and Difficulties Screen Form in order to make referrals to the Amundsen Care Team for students who:
  • experience trauma
  • exhibit symptoms of depression or anxiety
  • engage in substance use
  • are experiencing peer or family issues
  • exhibit anger that is beyond their control
 
If this is an emergency or crisis situation, Amundsen staff should follow school protocols.
